Case Management
Individual Case Management includes the building blocks toward positive benchmarks and outcomes
Our staff will work with physicians and specialist to develop an individualized care plan with your family member in mind. That happens with on-going review of care plans and outcomes to assess these patient advances. ​Patients admitted under this program will receive home health aide and skilled nurses for daily care.
